Key Responsibilities:

• Provide direct nursing care to residents including medication administration, wound care, and monitoring of vital signs.

• Assist in the development, implementation, and evaluation of resident care plans.

• Observe and report changes in resident conditions to the RN or physician.

• Document nursing care, resident status, and responses to treatment accurately in the medical record.

• Ensure safe and effective communication between residents, families, and the interdisciplinary care team.

• Maintain a clean and safe environment for residents and staff.

• Supervise Certified Nursing Assistants (CNAs) and delegate tasks appropriately.

• Participate in shift hand-offs, care conferences, and staff meetings.

• Comply with all federal, state, and local regulations, as well as facility policies and procedures.

• Respond to emergencies and provide appropriate care.

Qualifications:

• Valid and active Licensed Practical Nurse (LPN) license in [State].

• Current CPR/BLS certification.

• Prior experience in a long-term care or skilled nursing facility preferred.

• Knowledge of nursing practices, procedures, and regulations related to LTC.
